About the Addendum Notice

The critical execution criterion - the Bidder or its OEM (themselves or through resellers) must have executed projects for supply and installation / commissioning of same or similar Category Products during the preceding 3 financial years (current year and three previous financial years) as on bid opening, with one of - (i) Single order of at least 35 percent of estimated bid value, OR (ii) Two orders of at least 20 percent each, OR (iii) Three orders of at least 15 percent each. Satisfactory Performance Certificates from the respective Buyer Organisation must be uploaded with the bid.

Document Requirements
The bidder must upload audited balance sheets or CA / Cost Accountant certified turnover certificates for the relevant 3 financial years, OEM Authorization Certificate, satisfactory performance certificates from the Buyer Organisations for the executed similar orders, the drafted Malicious Code Certificate covering hardware and software, ISO 9001 certificate of the bidder or OEM, escalation matrix for service support, and documentary evidence of a functional service centre in the state of every Consignee location.
For Make in India compliance the bidder must align with the applicable DPIIT order. GST is to be checked by bidder on their own before quoting and reimbursement is capped at quoted GST percent or actuals (whichever is lower). Faulty Hard Disks of servers, desktops and laptops are not to be returned to the OEM / supplier under warranty (ESIC Security Policy).
Important Instructions
This Addendum supersedes all earlier Buyer Added ATC for the Neonatal Ventilator RC bid. Bidders must bid strictly as per this notice.
The execution criterion (1 order at 35 percent / 2 at 20 percent / 3 at 15 percent of estimated bid value in last 3 FYs) is non-negotiable; satisfactory Performance Certificates from the respective Buyer Organisation are mandatory.
The Bidder or OEM must possess ISO 9001 certification with a functional service centre in the state of every Consignee location (or set one up within 30 days of contract award).
Faulty hard disks under warranty will not be returned to OEM / supplier per ESIC's Security Policy.
