The bidder is also required to submit all duly filled Annexures mentioned in the ATC, along with the standard GeM bid documents.
Proof of Past Experience
For fulfilling the experience criteria, any one of the following documents may be considered as valid proof, as restated by the corrigendum - contract copy along with invoices with self-certification by the bidder that service or supplies against the invoices have been executed, execution certificate by client with contract value, or any other document in support of contract execution such as Third Party Inspection release notes.
Important Instructions

No bidder may quote a price lower than the estimated wages of Sanitation Officer or Sanitary Attendant; any such quote will be treated as non-responsive and rejected.
The price quoted for garbage disposal will be given priority in financial evaluation - L1 will be determined on the basis of this criterion.
Only the 10 machinery / equipment items listed in the revised Annexure-J may be proposed - no addition or substitution is allowed.
EMD via Account Payee Demand Draft must be in favour of "ESIC Hospital, Bhiwadi, Account no. 1" payable at SBI BHIWADI, with hard copy delivery within 5 days of bid end / opening date.
Service Provider must pay wages of contracted staff first and then claim from buyer with PF, ESIC and bank-statement evidence.
All ATC clauses in contravention of GeM GTC (or DPIIT MII Order, MSME policy, etc.) will be treated as null and void; any such bid may be cancelled by GeM without notice.
